*Membership dues/payments must be current to be seen by Dr. Hall, to receive prescription refills, to receive referrals, or to receive any paperwork.

*Insurance is NOT accepted or necessary to be seen at Access To The Hope Family Center. Medicare, Medicaid, and insurance companies will not be billed for services.

*All membership plans require a minimum of a ONE YEAR CONTRACT and are NON-REFUNDABLE.

A membership to Access To The Hope Family Center does not qualify as insurance to avoid tax penalties for not having required insurance.
